Saúde indígena: ONG envolta em polêmicas já recebeu R$ 2 bi do governo

“A serviço do índio para a glória de Deus”. É assim que se apresenta, no seu slogan, a organização não governamental (ONG) Missão Evangélica Caiuá (MEC), ligada à Igreja Presbiteriana do Brasil e hoje dona da maior cota do orçamento da União destinada a entidades sem fins lucrativos. Segundo dados do Portal da Transparência do governo federal, entre 2010 e 2017, a ONG abocanhou nada menos que R$ 2 bilhões em convênios firmados com o Ministério da Saúde para administração de distritos de saúde indígena no Brasil. Dos 34, a MEC tem nas mãos 18, espalhados de norte a sul do país.

Os valores repassados pelo governo à instituição impressionam. Em 2017, por exemplo, dos R$ 691,1 milhões empenhados pela pasta para a promoção da saúde indígena, R$ 421,9 milhões foram parar nas mãos dos missionários da Caiuá. Pelos dados da Transparência, a ONG foi a campeã no repasse de recursos a entidades sem fins lucrativos. No ano anterior, levou uma cota de R$ 348,8 milhões. No quesito “cifras”, ficou atrás apenas do Serviço Nacional de Aprendizagem Industrial (Senai) e do Centro Nacional de Pesquisa em Energia e Materiais.

Com o dinheiro, a Caiuá paga os salários de 9.391 funcionários que atendem, pelas suas contas, uma população de 426.158 indígenas nos 18 distritos sanitários sob sua tutela, fora a Casa de Saúde Indígena (Casai) do DF. A estrutura e os suprimentos, como material e medicamento, são fornecidos pelo ministério, via Secretaria Especial de Saúde Indígena (Sesai).

Quando abre os números, a ONG bate no peito para dizer que, dos quase 10 mil empregados, 5.167 são indígenas, sendo “grande parte” (15,7%) com nível superior. “A maior empregadora de indígenas do país”, destaca em negrito uma apresentação em slides da MEC, enviada à reportagem por e-mail. “Eles [os índios] nos ajudam muito”, afirma José Leopoldo Zanetti, coordenador regional da Caiuá em Dourados (MS), onde fica a sede da organização. “É uma conquista em conjunto”, acredita, sobre o que ele chama de “sucesso” da missão junto aos índios.

Transparente e conciliador

Para provar o argumento, a ONG apresenta dados da vigilância epidemiológica no distrito sanitário de Mato Grosso do Sul. Um gráfico mostra que, enquanto em 1999, ano em que foi “convidada” pela Fundação Nacional de Saúde (Funasa) para assumir a saúde indígena no estado, a mortalidade infantil nas aldeias batia na casa dos 140 a cada 1 mil nascidos, em 2015 esse número era de 18,9. Outra tabela, com a média de dentes cariados ou obturados em crianças de 12 anos, também depõe, em tese, pela excelência dos dentistas da ONG: os pequenos agora só perdem 1,87 dente, contra 4,3 em 2002. Tudo fruto de uma “parceria transparente e conciliadora” com os povos atendidos, prega.

A Caiuá foi fundada em 1928 por um pastor presbiteriano obstinado em levar a palavra de Deus ao povo Kaiowá do Centro-Oeste brasileiro. Em 1963, a organização abriu o Hospital e Maternidade Porta da Esperança, que prestava – e ainda presta – atendimento de saúde gratuito ao povo indígena. A “parceria” com o Ministério da Saúde, firmada via convênios, é de 1999, quando o Distrito Sanitário Especial Indígena de Dourados passou para as mãos da Caiuá.

Em 2003, juntaram-se a ele os distritos de Minas Gerais e Espírito Santo (MG/ES) e o do Maranhão (MA). Em 2009, já eram cinco distritos. Em 2011, depois que a Sesai foi criada e assumiu a atenção de saúde dessa população, o número de DSEIs sob as asas da Caiuá saltou para 17 porque, eles dizem, nenhuma outra ONG se interessou no chamamento feito pela secretaria naquele ano.

Em 2013, num novo chamamento, a Caiuá fechou o número que ostenta até hoje: 18 DSEIs mais a Casai-DF. O que sobra fica dividido entre outras duas entidades: a Associação Paulista para o Desenvolvimento da Medicina (SPDM), com nove DSEIs, e o Instituto de Medicina Integral Fernando Figueira (Imip), com sete.

O mais “caro” deles, segundo a própria Caiuá, é o distrito Leste, em Roraima, com um orçamento previsto de R$ 50,3 milhões para 2018, a fim de atender uma população de 41.685 indígenas. A maior, em pessoas atendidas, é o do Alto Solimões, no Amazonas, com 55 mil pessoas e orçamento de R$ 47 milhões para este ano.

A Caiuá está acostumada a rebater polêmicas envolvendo o seu nome. Tanto que, ao saber sobre a reportagem ao telefone, José Leopoldo chega a abrir um sonoro sorriso de conformidade. Vira e mexe, a ONG estampa notícias de jornal. Seja pela escalada fenomenal no ranking das entidades mais contempladas pela União, seja pelas denúncias de envolvimento de funcionários em esquemas políticos, atraso de salários e atendimento sucateado.

No final de 2017, dois indígenas do MS foram a um veículo de comunicação local dizer que até papel higiênico os pacientes do Porta da Esperança precisavam levar de casa. Em 2016, a denúncia era que a ONG estaria parcelando os salários dos empregados. Em abril do mesmo ano, a Caiuá já tinha sido condenada pelo Tribunal Superior do Trabalho a pagar R$ 5 mil a um ex-funcionário indígena que havia ficado 13 anos sem férias, sendo demitido e recontratado a cada ano, pelos términos e renovações de contrato da entidade com a Sesai.

Para além do sucateamento, os indígenas reclamam ainda do despreparo dos profissionais para atender sua população. O antropólogo Jósimo Constant, primeiro indígena graduado em antropologia na Universidade de Brasília (UnB), há um ano e meio se debruça sobre a saúde pública do seu povo, os Puyanawa, para sua tese de mestrado.

Eles são atendidos pelo DSEI do Alto Rio Juruá, no Acre, controlado pela Caiuá. “O recurso chega, mas é mal-empregado. E o indígena dificilmente denuncia, porque eles sequer sabem os seus direitos. O descaso é enorme. Eu digo isso como pesquisador e como indígena, com uma mãe que sofre com isso”, conta.

Jósimo diz que 14 etnias diferentes são atendidas pela estrutura do Alto Juruá. Além da dificuldade no atendimento, existem problemas pontuais, mas que, segundo ele, evidenciam a precariedade da atenção ao índio – como a falta de alimento tradicional daquele povo nos hospitais ou casas de saúde, quando o tratamento é prolongado.

José Leopoldo não nega que o hospital mantido pela ONG em Dourados esteja, de fato, precário. Segundo ele, no entanto, o orçamento de nove dígitos repassados pelo governo para a missão todos os anos nada tem a ver com o Porta da Esperança. O recurso serve única e exclusivamente para “gestão de RH”: contratar gente. Segundo o coordenador, o hospital, que tem 70 leitos, vive hoje com um orçamento de cerca de R$ 170 mil por mês – R$ 120 mil do governo federal, R$ 35 mil do estado, R$ 18 mil da prefeitura de Dourados e uma quantia “ínfima” de doações de igrejas e pessoas.

No rol das denúncias, a missão tem seu nome envolvido também em uma investigação da Polícia Federal contra Ismael Cardeal, coordenador da Caiuá em Roraima – justamente o estado que recebe o maior aporte de dinheiro dos convênios da entidade. A PF desconfia que Cardeal usou a ONG como cabo eleitoral quando concorreu a vereador em 2016. Ele acabou não sendo eleito, mas bem que tentou: teria oferecido empregos na Caiuá em troca de votos.

Briga política

José Leopoldo Zanetti, no entanto, culpa justamente os interesses políticos pelos falatórios sobre a ONG nos jornais. Acredita que o volume dos repasses do governo atraia atenção de partidos de esquerda e de direita que, frustrados, descontam seus aborrecimentos em denúncias de toda a sorte. O que Zanetti resume como “apanhar de tudo quanto é lado”.

“Sempre deixamos bem claro que somos uma instituição apartidária e temos assim permanecido ao longo de 90 anos de existência. Nesse sentido, são fortes as solicitação de atitudes de apoio e é justamente esse não atendimento que tem feito com que tanto a esquerda como a direita política fiquem aborrecidas.”

Em nota enviada por e-mail, o coordenador destacou que os recursos que mantêm o hospital “não se confundem com os recursos dos convênios que são específicos para a contratação de trabalhadores de saúde para atuar junto às comunidades indígenas” e ainda que a ONG “jamais” pode se beneficiar deles. Disse ainda que “nenhum centavo fica com a Missão” e que o único benefício da MEC é a “alegria de servir – por mais estranho que pareça nos dias atuais”, frisa.

O contrato da Missão com a Sesai vigora até 31 de dezembro deste ano e, segundo o Ministério da Saúde, não pode ser renovado. Em nota, a pasta informou que está “buscando uma solução integral para o atendimento aos povos indígenas”.

Para isso, o modelo de contratação das entidades de assistência deverá sofrer alterações a partir do próximo chamamento: as entidades, agora, precisarão estar vinculadas a uma universidade. “O objetivo é melhorar a assistência à população indígena na sua totalidade, com atendimento em saúde e infraestrutura, com amplo conhecimento das suas diversidades culturais”, diz o texto do ministério. Em 2018, o orçamento total da Sesai para ações de promoção, proteção e recuperação da saúde indígena é de R$ 1,4 bilhão.
